I have printed off fresh record check forms that are the correct ones. If you need one and it isn’t in your box, I’ve put some extras on top of the teachers’ station.
The new due date for receipts proving they are in-process is April 30th. Remember, they only accept certified cheque, money order. I believe that if you go in person, they also accept credit cards. I recommend calling. Cost is currently $65.
